These records will assist in contact tracing COVID-19 infections if required and WILL be stored confidentially and securely.

A REQUIREMENT FOR ENTRY TO THIS WORKSITE IS THAT YOU COMPLETE & SUBMIT THE FOLLOWING FORM.

A condition to entering this worksite you MUST confirm:

you have NOT been diagnosed with COVID-19

you have NOT been tested and waiting for a COVID-19 test result

you have NOT been in close contact with anyone with COVID-19

you have NOT got any symptoms consistent with COVID-19 (fever, cough, sore throat, shortness of breath, runny nose, chills or sweats)

you HAVE the appropriate PPE including face covering that must be worn at all times whilst practicable on site
